Waiver

I  confirm I am 21 years of age or older and I understand this is an important legal document related to my participation with Luminize Hypnotherapy. I agree to assume full responsibility for any risk, injury, or damages knowing or unknowing which I might incur as a result of participating with Luminize Hypnotherapy.

I knowingly and voluntarily waive all rights to any/all claims that I may have against Luminize Hypnotherapy and the practitioners and owners. I unconditionally release and forever discharge any persons, owners or their employees, agents, successors, subsidiary entities or representatives members or affiliates in any way to Luminize

Hypnotherapy from any and all actions, liabilities, damages, losses, costs, expenses, claims or demands including without limitation those based on negligence, gross negligence and breach of contract or breach of any statutory duty of care owed under applicable laws.

 

I have carefully read this document and I understand its contents. I am aware that this document is a release of all liabilities and is a legally enforceable contract.

Hypnosis is simultaneously a relaxed, meditative state and a state of focused concentration. During the Hypnosis session you may receive suggestions and directions that may influence the way you perceive certain tasks, foods, drinks, and people in the future.
By participating in this session you understand:
   •    That your perception may change and that you are doing so voluntarily.
   •    That results vary and that Luminize Hypnotherapy can not guarantee results. Hypnosis/hypnotherapy is not a replacement for medical treatment, psychological or psychiatric services or counseling.
   •    That the hypnotists do not treat, prescribe for, or diagnose any condition.
   •    That the hypnosis practitioner(s) are facilitators of hypnosis and are not practicing any other profession that requires a license under the laws of the State of Utah
   •    That in some cases it may be necessary for the practitioner to respectfully touch your shoulder, hand, forehead, or arm in order to assist you in relaxation. You give the practitioner permission and consent to do so in order to help establish a beneficial state of hypnosis.
